The vast majority of hemangiomas of infancy can be treated with watchful waiting. Parents should be re-assured. Photographs showing the natural history of hemangiomas of infancy are extremely useful to reassure parents. When hemangiomas of infancy are complicated by ulceration, they are capable of causing permanent deformity. Ulcerated hemangiomas of infancy and those that are life threatening should be actively treated. Oral systemic corticosteroids at doses of 2 to 3 mg per kg, for 2 to 10 months are the mainstay of therapy. Intralesional corticosteroids (Kenalog at 1-2 mg are utilized to assist with more rapid involution of the hemangioma. Interferon alpha and vincristine are considered for life-threatening hemangiomas or hemangiomas that fail corticosteroids. Interferon alpha is very effective but the neurological complication, spastic diplegia, limits its use to severe life threatening hemangiomas of infancy. Surgical excision can be an option for patients with ulcerated lesions or well localized hemangiomas. Pulsed-dye laser can be useful for infants with ulcerated hemangiomas.

The biopsy report should state the diagnosis, anatomic site, Breslow level, and whether biopsy margins are involved. Tumor thickness, as defined by the Breslow depth, is the most important histologic determinant of prognosis. The tumor is step sectioned. The section with the deepest level of penetration of tumor is used to measure thickness. An ocular micrometer is placed on the microscope. The pathologist measures the thickness of the tumor in millimeters from the top of the granular cell layer (or base of superficial ulceration) to the deepest part of the tumor. The report is given as Breslow level, followed by the depth reported in millimeters. The presence of ulceration microscopically, defined as the loss of epidermis overlying the melanoma, is the next most important histologic determinant of patient prognosis and should be used to upstage patients with melanoma when present. Ulceration or regression, if present, raises the possibility that the Breslow level may be underestimated. Clark's levels measure tumor invasion anatomically. The tumor depth is reported by anatomic site (ie, epidermis, depth in dermis, etc.) and assigned a Clark level of invasion. Clark levels appear to affect prognosis only in thinner (<1 mm depth) melanomas.

The lesion begins as a pearly white or pink, dome-shaped papule resembling a molluscum contagiosum or dermal nevus. The mass extends peripherally. The lesion may remain flat. Traction on the surrounding skin accentuates the pearly border. Telangiectatic vessels become prominent and easily recognizable through the thin epidermis as the lesion enlarges. The growth pattern is irregular, forming an oval mass and the surface may become multilobular. The center frequently ulcerates and bleeds and subsequently accumulates crust and scale. Ulcerated BCCs were formerly designated rodent ulcers. Ulcerated areas heal with scarring, and patients often assume their conditions are improving. This cycle of growth, ulceration, and healing continues as the mass extends peripherally and deeper. Masses of enormous size may be attained. BCCs may present as nonhealing leg ulcers. Biopsy specimens should be taken of leg ulcers that do not respond to treatment. The tissue mass of a nodular BCC has a distinctive consistency that can be appreciated during curettage or biopsy. It has poor cohesive forces and collapses or breaks down when manipulated with a curette. This is an important diagnostic feature that supports the clinical impression during the biopsy procedure.

The revised labeling now warns about three kinds of serious adverse events: • Gastrointestinal perforation, which may be fatal. This risk is increased in patients receiving concomitant anti-angiogenic agents, corticosteroids, NSAIDs, or taxane-based chemotherapy, and it is also greater in those with a history of peptic ulceration or diverticular disease. Tarceva should be permanently discontinued if the patient develops gastrointestinal perforation. • Bullous, blistering and exfoliative skin conditions, possibly including Stevens-Johnson syndrome and toxic epidermal necrolysis, which can be fatal. Tarceva treatment should be interrupted or discontinued if the patient develops one of these conditions. • Ocular disorders, including corneal perforation or ulceration. Tarceva should be interrupted or discontinued if the patient experiences symptoms of an acute or worsening eye disorder, such as eye pain.

Oral ulcers are also a common result of ceased cigarette smoking, affecting about two out of five quitters. Some drugs, such as nicorandil, have also been linked with oral ulcers.Some recreational drugs cause mouth ulcers.

Eradication of Helicobacter pylori infection led to permanent ulcer cure.
