  • Insidermedicine In Depth - March 22, 2011 A more sensitive test detecting injured heart muscle improves the ability to identify and care for patients who may have had a heart attack, according to research published in the Journal of the American Medical Association. Some signs that you may be having a heart attack and should seek immediate medical attention include: • Pain or discomfort in the chest, which may spread to other parts of the upper body or your stomach • Shortness of breath • Nausea and vomiting Researchers from the University of Edinburgh compared the outcomes of over 1000 patients who were admitted to hospital because of a suspected heart attack before the hospital switched to a more sensitive test for detecting troponin with those of over 1000 similar patients who were admitted after the new test was put to use. Troponin is a chemical that is released into the system when heart muscle dies. Before using the new test, a person was considered to have experienced a heart attack if their blood troponin level was more than 0.20 ng/mL. After implementing the new test, patients were considered to have experienced a heart attack if troponin levels greater than 0.05 ng/mL could be detected in their blood. Once use of the new test was established, the detection of heart attack increased by 29%. Also, the risk of dying or having another heart attack in the following year dropped by 58% among patients who had a troponin level between 0.05 and 0.19. These are the patients who would have had a negative result ...

  • Napolean Dynamite Rude Mood I haven't played the guitar since I started playing piano about two years ago. I finally decided to go in and get new strings for my acoustic and a new nut for my broken electric.....and somehow walked out with a new acoustic-electric... It might be hard to hear it....but behind the clanking and missed notes is that song from the end of Napolean Dynamite, followed by a fat fingered ruining of SRV's Rood Mood. I'm a little rusty from not playing for a few years. Also, I'm really bad at the guitar, so that doesn't help. Also, my pet's head fell off and I lost my job just minutes before. Additionally, an earthquake happened to be rumbling as I play. Around the midway point of the song, a group of Amazon tribesmen began blowing poisonous darts at me while a baby cried next to the barbershop quartet that was singing "baby on board" from the b-sharps in front of the guy repeatedly starting and stopping a chainsaw, so that didn't help either. I was also flat from low carbs.

  • "Know Your Heart Series", Diagnostic Testing, Blood Tests There are Fundamental Blood Tests that are important in order for the Cardiologist to assess an individuals heart health. These tests include a Lipid Panel, which measures Cholesterol, High Density Lipoprotein, Low Density Lipoprotien, and Triglycerides. The Complete Blood Count is of importance, in that it can indicate whether an infectious process is occuring in the body, via an abnormally high White Blood Cell Count. In addition, an abnormally low Hemoglobin/Hematocrit, can be consistant with Anemia. Anemia can induce Ischemia in the Heart, due to the fact that there is inadequate Oxygen transporting capacity of the Red Blood Cells. A Metabolic Panel is an important blood test, in that it includes the Heart Electrolytes, Potassium, Sodium, Calcium and Magnesium (sometimes Magnesium must be ordered seperately). In addition, the Metabolic Panel includes the Renal Function (Blood Urea Nitrogen/BUN, and Creatinine, Effective Glomerular Filtration Rate/eGFR). Abnormal kidney function effects the filtration of the blood, including electrolyte levels, which, if abnormal, could cause a Heart Rhythm Disturbances. Lastly, the Serial Cardiac Enzymes are important, in that the Troponin I is the most sensitive measure of damage to the heart muscle cells. The Troponin I is released upon the death of heart muscle cells (the Myocyte) into the blood-stream. The abnormally high Troponin I, and abnormal changes in the EKG, are clinically suggestive for a Myocardial Infarction. Also ...

  • Recall of Abbott Troponin- I Immunoassay (Aug. 2007) Abbott Laboratories is conducting a voluntary recall of all lots of the company's ARCHITECT STAT Troponin-I test because of a small number of complaints of inconsistent or invalid results at very low levels of troponin. Some of the invalid results were falsely elevated and some were falsely decreased. Patient treatment could be affected in either case. A falsely elevated result could lead to needless treatment of a non-existent heart attack, and a falsely decreased result could indicate that the patient had not experienced a heart attack when in fact an MI had taken place. Laboratories can continue to use this product, but should follow the updated instructions that Abbott has distributed. The company is advising laboratories to be cautious when reporting results at or near the lower limit of detection. They should also advise physicians ordering the tests about the possibility of inaccurate results at those levels, and to re-test in certain circumstances. For more information, laboratories can call Abbott Customer Support at 1-877-422-2688. Additional Information: FDA MedWatch Safety Alert. Abbott Laboratories Architect Stat Troponin-1 Immunoassay. • Sliding Filament Theory Part 1 NSCA Certification I made this video to help prepare me for the NSCA CSCS/CPT Certification. The sliding filament theory will have different phases depending on what textbook you are using to study. For this tutorial I am using the essentials of strength training and conditioning. Even if you are not studying for the NSCA certification, I think there is some good overlap in the material presented. Here is part of the outline to go along with the video: Muscular Contraction Muscle fibers contract by a shortening of their myofibrils due to actin filaments sliding inward over myosin filaments which causes the muscle to shorten and develop tension. During Muscular Contraction Z-lines are pulled toward the center of the sarcomere H-zone shrinks I-Band shrinks A-Band remains the same length Z-line Thin dark line of structural proteins running through the middle of the I-band. Thin filaments (actin, troponin, and tropomyosin) are anchored at each end of the sarcomere by z-lines. Link to the full outline :

  • What to Do about False-Positive Troponin Test Results(Jan06) FDA has issued a Safety Tip about the possibility of false positive results when troponin testing is used to help diagnose acute myocardial infarction ie, cases where the troponin immunoassay test is positive in the absence of AMI. False positive troponin tests are rare, but they can have serious consequences, because they can cause clinicians to suspect an acute MI when in fact none has occurred. The Safety Tip lists the factors that can interfere with accurate ***ysis, including the presence of clots and certain antibodies in the patient's blood sample, and it gives methods for reducing or removing interfering antibodies from the sample. It notes that lab personnel should suspect the presence of interfering antibodies when the test results don't agree with the patient's clinical presentation or when they're not reproducible. The Safety Tip also provides both clinicians and lab personnel with steps they should take if the troponin test doesn't match the patient's clinical presentation for AMI. For example, lab personnel should determine whether the patient is on anticoagulant therapy, examine the specimen for particles and clots, and re-run the specimen after doing a serial dilution to see if the results are linear. Clinicians should consider repeating the troponin test, as well as performing additional diagnostic testing. They should bear in mind that other clinical conditions may cause an elevated troponin level, and that the troponin test is only one tool among many ...
  • Ruling Out Myocardial Ischemia in Low Risk Chest Pain Patien In the emergency room, one million patients are evaluated annually for chest pain. Unfortunately, 2 to 5 % of them are sent home despite their having an acute myocardial ischemia (MI) or heart attack. • False Negative Results with VITROS Troponin I Reagent Pack Ortho-Clinical Diagnostics has notified healthcare professionals about a recall of the company's VITROS Troponin I Reagent Pack because of the potential for false negative results when troponin levels are only slightly elevated. A false negative might indicate that a patient had not experienced a heart attack when in fact an MI had actually taken place. The affected lot numbers are 3151 and 3170. Clinical labs who have these lots have been instructed to stop using the product, and notify physicians who have ordered the test in recent weeks. Labs with questions on this recall can contact the company at 1-800-421-3311. Additional Information: FDA MedWatch Safety Alert.
