Lumbar Sympathectomy Lumbar sympathectomy is being done under general anesthesia. It involves 3 small cuts for the endoscopic approach into the retro-peritoneal area. Once the surgeon is there a space is developed and the sympathetic chain at the lumbar region is visualized. Once it is visualized it is either excised or clamped at the level L3-L4. This particular procedure is done on both sides. The operation takes about an hour to perform and in contrast to the endoscopic thoracic sympathectomy the patient should stay over at the hospital for one day.

Hyperhidrosis- Neri Cohen, MD, Phd, GBMC Neri Cohen, MD, PhD Chief of Cardiothoracic Surgery Hyperhidrosis (Excessive Sweating) Treatment: Endoscopic thoracic sympathectomy (ETS). In severe cases, a minimally-invasive surgical procedure called sympathectomy may be recommended. The procedure is usually performed on patients with excessively sweaty palms. It is not as effective on those with excessive armpit sweating. This surgery turns off the signal which tells the body to sweat excessively. ETS surgery is done while the patient is asleep under general anesthesia. The doctor makes two or three tiny cuts under the arm. The patient's lung is collapsed so the surgeon has more room to work. A tiny camera, called an endoscope, is inserted to guide the surgeon to the appropriate nerve. After the nerve associated with the overactive gland is identified, it is removed or destroyed. The lung is restored to normal, and the wound closed with stitches. The identical procedure is done on the other side of the body. The surgery takes about a half hour. Patients usually go home the next day, but may experience pain for about a week. ETS requires special training. Before having this surgery, make sure your doctor is properly trained. Risks include artery damage, nerve damage, and increased sweating. New sweating occurs in about 50% of patients.

This is the CT of a 43 year old male patiënt with infected necrotizing pancreatitis that will undergo a VARD procedure; Video-Assisted Retroperitoneal Debridment. Two weeks before this procedure two large bore percutaneous drains were placed in the peripancreatic collection. The patient i...s placed in supine position with the left side 30 degrees elevated. A 5-7 cm subcostal incision is made in the left flank. With help of CT images and by following the percutaneous drain, the subcutaneous tissue and the fascia are dissected and we enter the retroperitoneal peripancreatic collection. First, with a regular suction device any pus encountered is removed. Two long sympathectomy hooks are inserted in order to keep in the incision open. We than insert the zero degree laparoscope. The first necrosis encountered is removed under direct sight with the use of long grasping forceps.
