Axons from the 2nd order neuron cross immediately via the ventral white commissure to the anterolateral quadrant of the spinal cord then ascend as the spinothalamic tract to the ventral posterior lateral nucleus (VPL) of the thalamus. The axons of the 3rd order neurons project to the postcentral gyrus or somatosensory cortex (there are also projections to the insular and anterior cingulate cortex but we are mainly focusing on the primary somatosensory cortex).
  • Querying the Shimmering Database I'm still trying to find a way to enrich my understanding of this wierd dual nature of being in which experience seems to emerge at the interface between the knower and the known (or something like that). Here I return to a metaphor I have used before of the world of noumena as a kind of 'shimmering database', which, like all databases, is not readable by humans but can be queried and the results of those queries presented in a number of forms. In this ***ogy the querying is carried out by the somatosensory system (which is itself not directly ascertainable) and the results of this querying produce the phenomenal experience of the lifeworld.
  • Diminishing Feeling of Being William James described emotions as products of the body's responses to external or internal stimuli, an idea developed (I think) in Merleau-Ponty, who said that emotions were 'perceptions of bodily states' (or words to that effect). Antonio Damasion has revisited this idea more recently, attributing not only the grand emotions of fear, anger, lust, joy etc to the workings of the somatosensory system but also the routine background sense that characterises our everyday 'feeling of being'. Returning to James, he invites us to imagine what if would be like to be in the throws of a strong emotion, anger for example, and have the bodily representations of that anger, the quick breath, the elevated pulse etc, disappear one by one. He asks whether, in the absence of these somatic behaviours, it would be meaningful to say that we felt 'anger' at all, and concludes that it would not. The feeling of anger is the consciously felt totality of these behaviours. If routine consciousness, apparently unruffled by the storm of high emotion, is also the feeling of what happens when nothing much is happening, then it would be interesting to imagine if the somatic behaviours accompanying this default state were removed sequentially. Would we cease to have consciousness at all? Would it make a difference if these senses were removed in any particular order?

  • NeuroLogic Exam Videos : Somatosensory Tracts Somatosensory Tracts Now let's add the 2 ascending sensory systems that give us important clinical information for localizing lesions in the neuroaxis. The first system is the spinothalamic tract (pain and temperature) diagramed in light blue and the second is the Dorsal Column- Medial Lemniscus system (discrimatory touch and position sense) outlined in dark blue. Two important anatomical (and hence clinical) points about these two systems: 1. The spinothalamic tracts cross almost immediately upon entering the cord but the Dorsal Column tracts don't cross until they reach the level of the medulla. 2. The course of these two sensory systems have a different "x" location until they reach the rostal pons where they are then in close proximity to each other for the remainder of their climb to the thalamus and on to the sensory cortex. The clinical importance of these two anatomical facts will be become apparent as we discuss the sensory exam.

  • Normal Sensory Exam ; Two-Point Discrimination Two-Point Discrimination Two-point discrimination is tested by using calipers or a fashioned paper clip. The smallest and most dense sensory units are located in those areas that have the greatest somatosensory cortical representation. The patient should be able to recognize two-point separation of 2-4 mm on the lips and finger pads, 8-15 mm on the palms and 3-4 cm on the shins.
  • Virtual Senses It seems likely to me that the sense-making processes which we have operate by taking inputs from the various sensory organs (interoceptive and exteroceptive) and then performing fairly scripted processes on those inputs (possibly corresponding to something like the 'cognitive operators' that Newberg and D'Aquili talk about). It also seems likely that, in addition to the literal organs of sense which we use to navigate the world, we also have 'virtual' senses which perform similar functions but are written in the software of the brain rather that the hardware of the body. Something like this must be the case if we are to account for dreams and other act of imagination. Putting these two ideas together, it may be possible that the cognitive operations which silently and routinely act upon the inputs from our literal somatosensory system also act within the loop of virtual cognitive apprehension. (Or maybe not).
