- Trehan's Total Peritoneal Excision Technique for Endometriosis Whist pelvic peritoneal excision surgery is an effective surgical option for treating endometriosis and is available in some hospitals, it has a recurrence rate of upto 40%; this may often result in the need for repeated excision operations or in the removal of the ovaries and uterus. The technique, 'Total Pelvic Peritoneal Excision', which Mr Trehan innovated is an extension of the above excision technique where the entire skin (peritoneum) of the pelvis is removed removing both clinical and subclinical endometriosis rather only clinical endometriosis. Mr Trehan believes and his results suggest that endometriosis is not a localized disease, but is a generalized disease of the pelvic peritoneum and a radical approach like total pelvic peritoneal excision is the answer. The theory of Total Peritoneal Excision (whereby the total removal of pelvic peritoneum (skin) removes all the present and future potential pelvic sites of endometriosis) is supported by the commonly believed theories for the causation of endometriosis (Retrograde menstruation, Coelomic metaplastic, and Migratory dislocation theory.) Fluid is introduced through a permanent tube in the abdomen and flushed out either every night while the patient sleeps (automatic peritoneal dialysis) or via regular exchanges throughout the day (continuous ambulatory peritoneal dialysis). PD is used as an alternative to hemodialysis though it is far less commonly used in many countries, such as the United States. It has comparable risks but is significantly less costly in most part of the world, with the primary advantage being the ability to undertake treatment without visiting a medical facility. Overfill, or increased intraperitoneal (IIP) volume, occurs when there's more fluid in the abdomen than was prescribed. This may result in serious injury or death from a number of conditions, including abdominal wall or diaphragmatic hernias, hydrothorax, heart failure, acute hypertension, pulmonary edema, decreased pulmonary function, pericardial effusion, and peritonitis. Over the last two years, Baxter has received reports of serious injuries and at least one death associated with overfill. Children may be more likely to experience this problem because of their smaller size. Critically ill patients and those with pulmonary and hemodynamic instability may also have a higher risk. Baxter alerted patients and clinicians to this problem in a January letter. Although Baxter is not removing the HomeChoice systems from the market, clinicians should weigh the risks and benefits of continuing to treat their patients with these devices.
