Had some intermittent CP. Presented to the ED with SOB worsened X 5 days, coughing, sore throat, and difficulty swallowing. ED physician's diagnosis: Pneumonia. Patient received first dose of antibiotics prior to admission. Patient admitted to cardiac telemetry step-down ward and then to CCU. Urgent 2D transthoracic Echo performed at the bedside. There is a very large circumferential pericardial effusion with early diastolic collapse of the right ventricle and late diastolic collapse of the right and left atria. The heart exhibits a severe pendulous motion within the pericardial sac. Observations are consistent with significant hemodynamic compromise and support the clinical diagnosis of cardiac tamponade physiology. Underwent an urgent large volume pericardiocentesis with approximately 970 ml of bloody pericardial fluid aspirated. The patient tolerated the procedure well and reported improvement of symptoms despite some persistent SOB post procedure. At time of discharge patient was diagnosed with stage 4 lung CA with pericardial involvement and subsequently he died about 20 months later.
