The Healthy Exercise for Lymphoma Patients trial, a three-year study led by Courneya, published last month in the Journal of Clinical Oncology, found that a regimen of aerobic exercise training produced significant improvements in physical functioning and overall quality of life benefits in patients with lymphoma. Researchers recruited 122 patients with Hodgkin's and non-Hodgkin's lymphoma, then classified participants by disease type and treatment status, whether they were undergoing chemotherapy at the time or receiving no treatments. Participants were randomly assigned to an exercise program designed to maximize cardiovascular fitness or to usual care, which did not include an exercise component. "The exercise program consisted of interval training," said Courneya. "We had patients ride the bike at a modest intensity, interspersed with high-intensity bouts of exercise, where they would go full out, exerting maximum effort for a minute or two at a time, then rest for a few minutes before doing it again. Mycosis fungoides and the Sezary syndrome are cutaneous T-cell lymphomas. These are characterized by the expansion of a clone of malignant CD4 helper T-cells that have a propensity to home and accumulate in the skin, lymph nodes, and peripheral blood. Confirming the diagnosis of mycosis fungoides is difficult in the early stages of the disease. The clinician will biopsy patch stage lesions and ask the pathologist to rule out MF. The classic features of the disease are usually absent at this stage. Typically a few atypical lymphocytes are seen and the pathologist reports an atypical lymphocytic infiltrate, suspicious for MF. Biopsy in patch-or-plaque stage shows a lymphocytic infiltrate in the superficial dermis, with individual lymphocytes migrating among epidermal keratinocytes. The affinity of the malignant T-cells to migrate to the epidermis is called epidermotropism. The lymphocytes are pleomorphic, hyperchromatic, and have hyperconvoluted nuclei. Staining of skin-biopsy specimens with a panel of lymphocyte markers is used sub classify the clone of cells. Immunohistochemistry and molecular genetics have demonstrated aberrant expression of the T lymphocyte surface antigens. - Lifestyle Factors Impact Survival of Non-Hodgkins Lymphoma Patients, Mayo Study Finds A new study led by researchers from Mayo Clinic in collaboration with six other US institutions has found that patients with non-Hodgkins lymphoma who smoked, consumed alcohol or were obese before their cancer diagnosis had poorer overall survival, compared to patients who did not have these risk factors. This association held after accounting for clinical and demographic factors, and also when considering only deaths due to this kind of lymphoma. For example, non-Hodgkins lymphoma patients with a 20-plus-year history of smoking had a 76 percent higher risk of death compared to never smokers; patients who consumed more than 43 grams of alcohol per week had a 55 percent higher risk of death compared to nondrinkers; and obese patients (defined as a body mass index of 30 or higher) had a 32 percent higher risk of death compared to patients with normal weight for their height. While smoking and obesity had already been found to increase the risk of developing non-Hodgkins lymphoma, this is the first US study to look at their role on survival after being diagnosed with non-Hodgkins lymphoma, the researchers say. For alcohol, they found that use was associated with poorer survival, which is opposite of the effect for developing non-Hodgkins lymphoma, where alcohol appears to lower risk.
