Dangerous Skin Reactions from Carbamazepine FDA is alerting healthcare professionals about dangerous and possibly fatal skin reactions to the drug carbamazepine in certain patient populations. The reactions have included Stevens Johnson syndrome and toxic epidermal necrolysis. Carbamazepine is an anticonvulsant used to treat epilepsy, as well as mania/bipolar disorder and neuropathic pain. This drug is sold under the trade names Carbatrol, Equetro and Tegretol, and also as a generic. These skin reactions are significantly more common in patients with a particular human leukocyte antigen allele called HLA-B*1502. This allele occurs almost exclusively in people with Asian ancestry, including South Asian Indians. Patients with this ancestry should be screened for the HLA-B*1502 allele before starting treatment with carbamazepine, using available genetic tests. If test results are positive, the drug should not be started unless the expected benefit clearly outweighs the risk of serious skin reactions. Patients who test positive for HLA-B*1502 may also be at increased risk from other antiepileptic drugs that have caused serious skin reactions. So clinicians should consider avoiding these drugs when other treatments are equally acceptable. Patients who test negative for HLA-B*1502 have a low risk of developing serious skin reactions from carbamazepine, but these reactions can still occur, although rarely. So healthcare professionals should watch for symptoms in any patient taking this drug. But note that over 90 percent ...

3D Medical Animation: Antibody Immune Response This 3D medical animation shows how antibodies stop harmful pathogens from attaching themselves to healthy cells in the blood stream. The animation begins by showing normal red and white blood cells flowing through the blood stream. Next, a single pathogen appears onscreen slowly moving toward its destination on the surface of a cell. The tubular extensions on the pathogen are surface proteins which attach to corresponding surface proteins on a white blood cell, or leukocyte. As the animation continues, more pathogens continue to attach to the white blood cell, rendering it ineffective. During the immune system response, Y-shaped antibodies begin attacking the pathogen, binding to its surface proteins as the pathogen attempts to anchor to the blood cell. Angel Darina. UPDATED 21.2.10 Darina was born on 15.06.2003. At the age of three she was diagnosed with rare form of leukemia. She was in need of at about 300 000 euro for a bone marrow transplantation in Israel - unaffordable amount of money for her family (and for almost any other family I guess). A charity campaign for donations was started and Darina touched so many hearts all over the country - thousands of people donated and the money were collected in at about 3 months. This was never seen before in Bulgaria. Dari had (and still have) the magic power to touch the hearts in a very specail way. So the money were collected and she went to Israel in time to receive her lifesaving treatment. The most suitable bone marrow donor was her mother (even she was not a perfect match) and on 15.03.2007 Dari underwent an successful bone marrow transplantation (as the professor said). For our great sorrow, unfortunately, Dari caught a flu-like infection, with a virus which would be harmless for a healthy human, but could easy kill a child whose immune system is so weak (due to the transplantation related immune supressing treatment). Two and a half months after the transplantation (on 30.05.2007), at the hospital in Israel, precious little Darina flew on her way to Heaven. No more pain for her tender body. Serious Skin Reactions in Asian Patients on Epilepsy Drugs FDA is investigating new preliminary data suggesting an increased risk of serious skin reactions from the anti-epileptic drugs phenytoin and fosphenytoin if they are taken by Asian patients who are positive for the human leukocyte antigen allele HLA-B*1502. These reactions include Stevens Johnson Syndrome (SJS) and toxic epidermal necrolysis (TEN). Phenytoin is marketed as Dilantin, Phenytek and generics. Fosphenytoin sodium is marketed as Cerebyx and generics. It is estimated that in parts of China, Taiwan, Thailand, Malaysia, Indonesia and the Philippines, 15 percent or more of the population may carry the HLA-B*-1502 allele. The frequency in South Asia, including India, is somewhat lower, and in Japan and Korea it is under one percent. It was previously established that another anti-epileptic drug, carbamazepine, increases the risk of these skin reactions in Asian patients with the HLA-B*1502 allele. FDA has recommended screening patients of Asian ancestry for this allele before prescribing carbamazepine and not starting the drug unless the expected benefit clearly outweighs the risk. In the case of phenytoin and fosphenytoin, the possible risk is still being studied, and there is not enough information at this point to recommend testing for the allele before starting therapy. However, FDA is recommending that physicians avoid using phenytoin as a substitute for carbamazepine in patients who test positive for the HLA-B*1502 allele. With carbamazepine, over 90 percent ...
