Typical Lesions The most typical, primary lesion of chondrodermatitis nodularis helices, is a firm, tender, red to pink, 2- to 4-mm papule with central scale and a central keratotic punctum. The punctum has firm, adherent crust or scale, resembling a small cutaneous horn. Removal of the scale reveals a small central erosion. The surrounding skin shows scale, actinic damage with atrophy and telangiectasia. Location on the ear This condition is classically found on the most prominent and lateral portion of the auricle. This can occur on the helix, but also the antehelix, depending on the shape of the ear. Occasionally, there may be more than one lesion. Differentiating from squamous cell carcinoma Unlike a squamous cell carcinoma lesion, the sides of the CNH nodule slope down from the center, are more firm, less necrotic and more tender. Squamous cell carcinomas of the ear are typically necrotic and may bleed or ulcerate. If there is doubt about the diagnosis, a skin biopsy should be performed. Inflamed lesions When actively inflamed, the base is red and swollen. The universal symptom is pain, described as stabbing and sharp. Any type of pressure is intolerable, and sleep may be affected if one is used to sleeping on the affected side.

A keratoacanthoma is a characteristic solitary flesh-colored to red, crateriform nodule, usually 0.5 to 2.0 cm in diameter. The lesion erupts rapidly and is often quite tender. A central keratotic plug or depression conceals a deep keratinous cavity. This plug or depression gives the nodule its characteristic volcano-like shape. The nodule is firm in texture, tender to palpation and pressure. Keratoacanthoma nearly always appears on sun-damaged skin. Typical locations include the face, neck, dorsal hands and sun exposed extremities. It occurs on the legs more often in women.

They are firm, keratotic lesion, usually found in sun-exposed areas. Actinic keratoses are precursors to squamous cell carcinoma, thus, they have malignant potential. They are caused by years of cumulative sun exposure, leading to DNA damage of keratinocytes. Roughly 5 to10% of actinic keratoses progress to invasive squamous cell carcinoma over several years. Although spontaneous regression of actinic keratoses can occur, the biologic behavior of each actinic keratosis cannot be predicted, thus, every lesion is treated as a precursor lesion. Individual actinic keratoses become progressively more common after 40 years of age and are more common in lighter skin types. They occur along with other signs of sun damage, such as atrophy, uneven pigmentation and telangiectasias. They are predominately found on the face, scalp, ears and neck, as well as the dorsal hands. The lower lip equivalent to an actinic keratosis is called actinic cheilitis.

Keratotic follicular plugs appear on the surface which are described as dells.

Dermatosis papulosa nigra is a term used to describe the seborrheic keratoses of the face seen more commonly in African-Americans. Dermatosis papulosa nigra lesions are 1 to 2 mm, dark brown keratotic papules concentrated around the eyes and upper cheeks, with an incidence of 30-35% in African-Americans. Stucco keratoses, sometimes referred to as barnacles, are common, nearly inconspicuous, papular, warty lesions occurring on the lower legs, especially around the Achilles tendon area, the dorsum of the foot, and the forearms of the elderly. The 1- to 10-mm, round, very dry, stuck-on lesions are considered by most patients to be simply manifestations of dry skin. The dry surface scale is easily picked intact from the skin without bleeding, but it recurs shortly thereafter. The lesions can be removed with curettage or cryosurgery but are most often left untreated. The sign of Leser-Treat is the sudden explosive onset of numerous seborrheic keratoses in association with internal malignancy. Most patients have metastatic disease when the keratoses appear. The SKs often parallel the course of the malignancy, decreasing in number and size following surgical or chemotherapeutic intervention and returning with recurrence of the cancer, but this is not always the case. Patients with numerous SKs need not be evaluated for malignancy unless the lesions erupt abruptly.
