Since then, the senior author has been directly involved in 23 such operations with one early death. At present there has been no late death. Variations in technique led us to the present preferred approach. 1) The full circumferential mobilization of aortic root minimizes tension on the coronary arteries, thus avoiding coronary artery re-implantation. 2) The division of ascending aorta allows Lecompte maneuver which protects bilateral pulmonary arteries from potential obstruction. 3) Nearly one third (7 out of 23 cases) did not need VSD patch as in this case in the video, when the right ventricular outlet wall below the aortic annulus covered the defect. 4) The large area from which the original aortic root was mobilized can be covered with a crescent shape patch and the proximal end of main pulmonary artery can be connected to this patch with an ample opening. The ASD should be closed in the usual manner. Since the operation asks for a prolonged aortic cross clamp time, the single dose Custodiol® cardioplegia was considered to be suitable for myocardial protection. The patient was a 14 month-old girl, who weighed 7.3 kg, and measured 72 cm in height (BSA 0.37). The aorta was cannulated high, just proximal to the origin of innominate artery with 10 Fr. cannula. Baby Pratham Yogesh Mehta, age 2 months, reg no ***X/ IPD*** was admitted to our institute on 10-Sept-08, with a diagnosis of cyanotic congenital heart disease, double outlet right ventricle, large VSD, Transposition of Great vessels, straddling tricuspid valve and cleft mitral valve and under went a arterial switch with mitral valve repair with PA Band on 12th September, 2008. The child had an extremely stormy intra operative course and failed coming of bypass twice and was finally shifted to the ICU with extremely high inotropic supports and open chests. The child was in persistent low cardiac output state for the next five days and only subsequently showed mild improvement and the chest was closed on 18th September, 2008. Later once the child was stable for next three days and effort was made to reduce the ventilator support but it failed recurrently. Subsequently the child developed severe abdominal distension, for which the child was evaluated and it was diagnosed to have paralytic ileus and an exploratory laparotomy was done on 2nd October and an ileostomy was done after resection of the diseased segment of distal ileum. Now the child is recuperating in the ICU and presently has mild renal failure with culture negative sepsis and is on 3rd generation antibiotics and total parenteral nutrition and is critical. If the child shows and improvement in the general condition, he is likely to have an ICU stay of minimum 3 weeks.

FDA will allow the firm to continue to provide routine service maintenance, or to replace components, parts, or accessories for the Colleague and Syndeo Infusion Pumps that were already in the hands of customers before October 12, 2005.   • Ozone Therapy With Simple Acess Fluid Extraction or Aquapheresis! SAFE or Aquapheresis is a device-based therapy intended to remove extra fluid from blood and body ; Simple Access Fluid Extraction (SAFE) as the name implies. This is an FDA approved therapy. Patients with heart failure typically are treated with various medications, including angiotensin-converting enzyme inhibitors, beta-blockers, diuretics, and inotropic drugs. Although these drugs don't stop disease progression, they may reduce symptoms and improve functional capacity. Over time, however, some patients stop responding to these therapies. SAFE , a process that removes fluid from the intravascular compartment, can help ease symptoms for these patients. It may even help patients respond again to conventional drug therapy. During SAFE, the patient intravascular fluid remains stable, as fluid shifts from extravascular space that lost from the vessels. This reduces edema and third spacing. But because fluid is not removed from the vascular compartment any faster than it can be recruited from the peripheral tissues, patients do not become hypotensive- the plasma refilling rate is adequate to prevent hypovolemia. The patient's heart rate, blood pressure and serum electrolytes also remain largely unchanged. In general, SAFE improves the quality of life with the ceasing of anginal pain and increase of functional abilities. It is used in conjunction with ozone therapies at our medical ozone clinic.
