The sebaceous hyperplasia lesion begins as a 1 or 2 mm soft, pale yellow to skin-colored, minimally elevated papule. With time, the lesion attains a maximum size of 3 to 4 mm and develops a central umbilication. Mature papules possess a distinctly yellow-orange color and are more sharply defined from the surrounding skin. Papules may be solitary but are more commonly multiple and scattered randomly on the forehead, eyelids, nose, and cheeks. Papules may yield sebum from the central umbilication with palpation. An orderly array of fine telangiectasias may radiate outward from the umbilication toward the periphery of the papule. Individual lesions may be confused with basal cell carcinoma, small keratoacanthoma or molluscum contagiosum.

Sebaceous hyperplasia is a common benign condition consisting of prominently enlarged, sebaceous glands on the face. Sebaceous hyperplasia occurs in both men and women. Most lesions represent a single, hypertrophied sebaceous gland with multiple lobules arranged around a central enlarged sebaceous duct. Lesions occur in all skin types but are more easily seen in lighter skin. The etiology of sebaceous hyperplasia is unknown. Genetic inheritance most certainly plays a large role. Sun damage has been suggested as a contributing factor. The lesions are entirely asymptomatic but persistent. Papules can become disfiguring and are mostly of cosmetic concern. Older patients are typically concerned that the lesions represent basal cell carcinoma or other type of skin cancer.

Generally speaking, these lesions will increase in number over time. Rarely, some lesions have been reported to resolve on their own, but the vast majority tends to remain permanently. These represent an overgrowth of the sebaceous glands (oil producing glands) on the face. Their growth is associated with exposure to different androgen hormones and sun. This explains why sebaceous hyperplasia is seen more commonly in men than in women, but it is still seen in all people. Sebaceous gland hyperplasia is a harmless condition that is only a cosmetic concern. It does not represent anything bad, such as skin cancer. Some of the lesions, if extremely large in size and odd looking, may be evaluated for additional sebaceous gland disorders. Sebaceous gland hyperplasia can effectively be treated with a number of cosmetic procedures including minor surgery, electrosurgery liquid nitrogen freezing, and, most effectively, with laser therapy.
