  • Preventing Errors w/ Neuromuscular Blocking Agents (Aug. 06) ISMP recently issued a Safety Alert called "Paralyzed by Mistakes," which warned about inadvertently giving neuromuscular blocking agents such as pancuronium to patients who aren't receiving ventilator support. This can lead to respiratory arrest. Some patients have died or incurred permanent injuries as a result of these errors. ISMP notes that some of the errors are due to look-alike packaging and labeling. In one case, an ED nurse administered pancuronium instead of flu vaccine because the vials and labeling looked very similar. Look-alike drug names have also caused problems. In one case vancomycin was misread on a faxed medication record as vecuronium. Giving the neuromuscular blocking agent after the patient is extubated has resulted in serious problems, too. In one case, an infusion bag of vecuronium was left in a patient's room after ventilator support had been removed. The bag was later mistaken as potassium chloride infusion and administered to the patient. Serious patient injuries have also been caused by the opposite problem ie, administering the neuromuscular blocking agent too soon, before the patient is intubated. Some errors have resulted from preparing syringes from a multiple-dose vial and neglecting to label them. In one case, an unlabeled syringe of vecuronium ended up in a supply of saline flush syringes and was given to a 3-year old child. Unsafe storage is another cause of serious mistakes.   • Laparoscopy -- perforated DU and septic shock A 40 years old female recently diagnosed with Child C liver cirrhosis and portal hypertension was brought to the Emergency Department with an acute abdomen; she was in septic shock, anaemic (Hb 82), acidotic, hypotensive, tachycardic, tachypnoeic, hypoxemic, dehydrated, anuric, febrile, jaundiced (Brb 160), coagulopathic (INR 2.1), hypoalbuminemic (18); LFT's were elevated; on examination she was lethargic and had a grossly distended abdomen. She was rushed to the operating theatre and underwent a laparoscopy ( a Visiport was used through a right paraumbilical approach to create the pneumoperitoneum and three 5mm trocars to complete the procedure laparoscopically): free fluid, collections and fibrinous deposits were found throughout the abdomen; the large and small bowel was grossly distended; the liver was rigid with cirrhotic appearance and the veins in the falciform ligament were of impressive size; the gallbladder was also distended. After aspiration of the fluid and drainage of collections, the gallbladder was lifted and loops of small bowel were removed from early adhesions in the right upper quadrant; a punched out perforation was identified on the anterior wall of the duodenum. This was sutured with a figure of eight 2/0 Vicryl and patched with omentum; the abdomen was further lavaged and drained.   • USP Statement on Preventing Errors w. Neuromuscular Blocking In a previous show, we told you about a report from the Institute for Safe Medication Practices (ISMP) which warned about inadvertently giving neuromuscular blocking agents such as pancuronium to patients who aren't receiving ventilator support. This can cause respiratory arrest, permanent injury, or death. ISMP noted that some errors are due to look-alike packaging and labeling, and recommended placing warning labels on vials, syringes, infusion bags and boxes that say "Warning: paralyzing agent, causes respiratory arrest." ISMP also cited unsafe storage as a cause of errors, and they recommended keeping boxes containing these agents in separate refrigerators and shelves. ISMP had several other recommendations. For example, allow floor stock of these drugs only in the OR, ED and critical care units, require an independent double check of the drug against the actual order before dispensing and administering, and isolate vials, syringes and infusion bags containing the drug as soon as the patient has been extubated or the drug discontinued. Now the United States Pharmacopeia (USP) has issued a similar statement warning about these kinds of errors. The USP report lists over 20 recommendations to help solve this problem.   • In loving memory of my baby - For Matteo/Pour Matteo - Severe Brain Damage Mattéo Riopel 05/12/29 - 08/11/04 Mattéo was born at 33 weeks with a tracheo and bronchomalacia and severe reflux, which required him to have 2 surgeries. On Sept. 25, he had his last surgery to correct the gastro-oesophagal reflux at the Montreal Children's Hospital. On Oct 17, we came back to Chisasibi, James Bay (where we live) and following his latest surgery, Mattéo was limited in his amount of food intake so for the next 3-6 months he was to receive gavages. But he was basically ok and he went trick-or-treating for Halloween, like any other kid, dressed as an elephant. On Nov 2, he went to bed and he was fine. He woke up an hour later complaining of headeaches and vomiting. I brought him to the Chisasibi Hospital and things started to deteriorate from then on. He was intubated at 6am and brought by medical plane to Montreal. A few hours after arriving in Montreal, tests confirmed that he had a blood clot at the brain stem. On Nov 4, he had cerebral death, he was extubated that evening and died a few minutes later. It was very quick, painful and inexplicable. He will be missed, but never forgotten.   • NEW NOSE YAY #2 Just me and my face. bandaged. bleeding. I talked too much after surgery and I think it made me bleed more... I was super conscious after the anesthesia wore off (maybe a little loopy) so I was very talkative. I love my friends who took me there and I love the doctors and the nurses I am so glad I finally did this. I feel no pain just slight burning/stinging sensation- trying to hold off on pain meds as long as possible. I woke up while being extubated which was pretty cool.... I remember being moved from the operating table to the bed thing on wheels, and then after that nothing, and then I woke up and was pretty wide awake from being put under for a little over two hours. Only thing I'm worried about is that the plaster is coming loose on one side of my cast... I might have to go in and get a new one put on because this thing is kinda bloody and loose. My nose seems like it has a low profile like I wanted!! I'm so happy! I'll talk more about the surgery when I can talk more, but I'm trying to stay silent until the bleeding stops

We chose to continue the pregnancy and prepared ourselves for fetal demise (if delivered before 24 weeks gestation) or for a severely disabled child (if born before 28 weeks). Although I never replenished amniotic fluid and my AFIs remained critically low (0-2.5 cm max), the pregnancy continued for another 12 weeks as I remained hospitalized on bedrest for 90 days until I went into labor at 31 weeks and 6 days. We knew that even though we'd made it gestationally farther than any docs thought possible, chances were high his lungs would be hypoplastic at birth and we would lose him in a matter of hours post-delivery. Born by emergency c-section, Bennett was unable to breathe at birth, diagnosed with Pulmonary Hypoplasia, was intubated and put on a high-frequency ventilator.   • MY MIRACLE BABY admission diagnosis: Cardiopulmonary arrest aspiration discharge diagnosis: esophageal reflux Hospital Course Summary: At 19 days of life. patient became apneic and pulseless following coughing during feeding, in the setting of a 5 day history of URI symptoms. Patient was resucitated with approximately one minute of chest compressions and rescue breathing. He was taken by EMS to Rady Children's Hospital where he was noted to have coarse breath sounds and retractions refractory to suctioning and nebulizer treatment. Therefore , patient was intubated , cultured for rule-out sepsis. and started on empiric ampicillin and cefotaxime. Ptient was transferred to Naval Medical Center San Diego and monitored in the Pediatric ICU. CXR showed a R perihilar opacity. Head CT showed no acute intracranial abnormality but MRI showed evidence of left occipetal infarction. EEG showed spikes that could be within normal limits. Neurology recommended no further intervention unless patient demonstrated symptoms. Patient showed no focal neural deficits during his hospital stay and acted appropriately. Therefore, no further work-up was done. Patient did well during his stay in the PICU. He had no apneic events. He tolerated NGT feeds well. Extubation was attempted on day 4 of admission but patient failed and required re-intubation. Patient was successfully extubated on day 7 and transferred to the pediatric ward.
