As the injury progresses forward into Stage 2 the talus rotates further laterally putting tension on the anterior inferior tibiofibular ligament resulting in either a rupture of this ligament or an avulsion fracture off the anterior portion of the fibula or tibia. As the forces continue past stage 2 rupture of the interosseus membrane begins to occur distally at the level of the anke joint. The exit point of this rupture creates a Stage 3 injury, described as a high short oblique fracture of the fibula beginning above the level of syndesmois. This fracture can be at various levels on the fibula. The most proximal fracture pattern occurs at the head and neck of the fibula which is described as a Maisonneuve fracture. If the force progress into Stage 4 either a rupture of the posterior tibio-fibular ligament or a fracture of the posterior malleolus (Volkman's fracture) will occur.   • Physiopathology Aortic dissection Physiopathology of acute aortic dissection: an intimal tear occurs on a weakened aorta (cystic medial necrosis due to arterial hypertension or to dysplasia of connective tissue as Marfan syndrome) leading to the separation of the layers within the aortic wall by the blood flow. The result is the creation of a false lumen (double-barreled aorta) and the propagation of the dissection. This false lumen extends proximally, resulting in a an aortic insufficiency, or distally towards the remaining aorta and collaterals. This false lumen can be compressive on arterial branches (coronary arteries, supra-aortic arteries, visceral arteries, limbs arteries....) leading to ischemia in the interested territory (malperfusion). The false lumen increases in diameter because subjected to systemic pressure. Spontaneous evolution is towards tamponade (pericardial effusion of blood), either progressively or acutely by aortic rupture, leading to death.   • Morton's Neuroma Pain in Foot - Surgical Excision Resources Consultant: Kelly Powers Producer: Nicholas Giovinco A Morton's neuroma is a pathological condition which is caused by a benign neoplasm of the plantar interdigital nerve in the foot. This results from a tumorous growth of the perineurial tissue called a fibroma, thus a Morton's neuroma is not a true neuroma. Although a Morton's neuroma most commonly arises in the 3rd interspace, between the 3rd and 4th metatarsal heads, it may also be found in the 2nd or 4th interspace as well. Symptoms of a Morton's neuroma may include numbness, paraesthesia, and a sharp, shooting pain, which radiates towards the affected toes. This sensation is elicited or exaggerated by compressing the metatarsal heads together which, in turn, compresses the enlarged nervous structure. Conservative care of a Morton's neuroma may include sclerosing and anti-inflamatory injections, as well accommodative orthosis. Although these methods may relieve painful and immobilizing neuroma conditions, surgical intervention may be required. This involves soft tissue exposure or dissection of the enlarged nerve body. Once this is done, the neuroma must be isolated and removed by cutting away the stem, proximally, and the branches, distally. Because the nerve is removed, there will be a loss of sensation in the affected area for several months or even years. Over time, this area will re-innervate by means of communicating branches from surrounding nerves.
