  • Clonus Significant and multiple beats of clonus in the left foot of a patient also found to have a positive Babinski (upgoing toes). This is a male patient. Sorry the cell phone camera doesn't do this justice. Update (just before mid-2007): This patient has cervical spinal c*** stenosis with myelopathy with subsequent upper motor neuron findings as you can see. He is scheduled for spine surgery to hopefully correct the compression of the spinal cord and resolve some of his neck pain and radicular symptoms. **Update as of March 2008: This patient ultimately had significant cervical stenosis causing his upper motor neuron signs but did not have his operation (for various reasons which cannot be described to protect patient privacy). He fell, about a year after these videos, and suddenly could not move all 4 extremities. He was taken for emergent decompressive surgery for his cervical spinal cord. Despite rapid intervention, he is now suffering from a central cord syndrome: he can move his legs better than his arms, but in his case, he is functionally a tetraplegic (quadriplegic - the old term). See for an article on central cord syndrome.

  • Refined Approach to Back Surgery Helps Elderly Ninety-year-old Charles Borenstein loves playing golf but had to put his clubs away when back pain became so bad he could hardly stand. He thought his life was over. Santiago Figuereo, MD, FAANS, medical director of the Miami Neurological Institute () in Aventura, Fla., diagnosed Mr. Borenstein with lumbar stenosis, a degenerative condition of the spine. Dr. Figuereo determined surgery was the only way to alleviate the pain. He says that with lumbar stenosis, the spinal column narrows. Specialty trained neurosurgeons like Dr. Figuereo can surgically go in and open it back up again. Elderly patients require extra special care. Dr. Figuereo has fine-tuned a minimally invasive procedure called a decompressive laminectomy, which is done through a small incision in the back. "This procedure is typically done to address one, maybe two discs. Using special tools and a laser-beam focused camera, I'm able to reach as many as five discs in one procedure," Figuereo said. The small incision avoids the need to cut muscle, reducing risk and increasing recovery time. Just four weeks after surgery, Borenstein was back on the golf course.

  • Live Webcast: Lumbar Laminectomy A lumbar laminectomy, or lumbar decompression, is performed to treat spinal stenosis, a narrowing of the spinal c*** that puts pressure on the nerves in the lower back, which causes symptoms in nearly 500000 Americans over the age of 50. Used to manage related conditions involving deformities of the vertebrae, such as spondy***sthesis or scoliosis, a transforaminal lumbar interbody fusion (TLIF) is an innovative, sometimes safer, more efficient alternative to other anterior and posterior-approach fusion procedures. Both conditions cause pain and weakness in the lower back and legs, leading to walking difficulty in many patients. The conditions occur as people age because ligaments around the spine thicken and the discs, or cushions, between the vertebrae start to deteriorate. During the surgery, an incision will be made in the patient's back, but surgeons will approach the spine from the side, minimizing the nerve manipulation required to access the vertebrae, discs and nerves. They will remove bone and ligament from the spine to open the spinal c*** and relieve pressure on the nerve roots which has been caused by stenosis. Next, the surgeons will perform a TLIF to fuse the spine where one vertebra has slipped forward onto the vertebra below it. (This generally occurs with spondy***sthesis, but in the case of the webcast patient it has been caused by complications from scoliosis.) Some vertebral bone will be removed to reduce nerve exposure, and then the pain-causing ...

  • Doing My Own Carpal Tunnel Surgery (Dr Henry David Nava Dimaano, FPOA) Finally did surgery on my problematic right hand last Saturday, Jan 9th. It's been persistently numb since Christmas, due to all the manual work I've been doing for our new house. I've been having this numbness -- thumb, index finger, long finger, & the thumb-side half of my ring finger -- since 2007; but back then, I only felt it whenever I'd be biking for more than 15 minutes straight. (I ride a mountain bike a lot; here in Cebu, it's how I haul my ass from home to school & back.) The symptoms never persisted beyond that. Until now. So, time to save my right median nerve from utter compressive damnation. (I definitely don't want to end up like my dad, who never had decompressive carpal tunnel surgery until it was too late. He was misdagnosed & mal-advised for his carpal tunnel syndrome problem back in 1999 -- both his hands have atrophied thumb muscles now, no thanks to the idiots who told him that "it's nothing sir, just exercise it regularly to counteract the weakness & the ulnar mono-neuropathy". They could have at least gotten the nerve right. Too bad I became an orthopedic resident 6 years too late!) Next up, my left carpal tunnel -- if it ever flares up like my right. Yeah, I get numbness of the left hand median nerve distribution too, but it's more transient, more limited to the biking thing. But eventually, I will have to undergo surgery for the left side as well. Oh yeah, I also did a trigger finger release of my right ring finger -- that's been locking ...
