  • Lichen Planopilaris Treatment Dr. Diane M. Hoss, MD discusses Lichen Planopilaris Treatment. See more at PLEASE RATE AND COMMENT!!! Of all the alopecias, both non-scarring and scarring, lichen planopilaris is the most difficult to treat. There have been no well-controlled trials documenting the benefit of any therapy in LPP. Since there is no possibility of regrowth in the scarred areas, the goal of treatment is to prevent enlargement of the scarred areas and to prevent new areas of inflammation from developing. Several treatments have anecdotal support and are commonly tried in patients with LPP. These include superpotent topical steroids, intralesional steroids, and short courses of oral steroids. Doxycyline and tetracycline have been tried because of their anti-inflammatory effects. Plaquenil has been used because of its benefit in patients with discoid lupus erythematosus, another lymphocyte-mediated scarring alopecia. Isotretinoin, dapsone, and thalidomide have all been tried. The biologics are also being tried for LPP. In my experience, LPP waxes and wanes over time, probably without regard to treatment. At times, there seems to be improvement that coincides with initiation of therapy, but then the disease may worsen or new areas develop while on this same therapy. It is likely that no therapy has been found that can influence the course of a patient's LPP. Over time, LPP seems to burn out. This may take many years to occur and there may be widespread scarring alopecia before ...

  • Facing Leprosy Photographer Jan-Joseph Stok Leprosy or Hansen's disease (HD), named after physician Gerhard Armauer Hansen, is a chronic disease caused by the bacteria Mycobacterium leprae and Mycobacterium lepromatosis.Leprosy is primarily a granulomatous disease of the peripheral nerves and mucosa of the upper respiratory tract; skin lesions are the primary external sign. Left untreated, leprosy can be progressive, causing permanent damage to the skin, nerves, limbs and eyes. Contrary to folklore, leprosy does not cause body parts to fall off, although they can become numb and/or diseased as a result of the disease. Leprosy has affected humanity for over 4000 years, and was well-recognized in the civilizations of ancient China, Egypt, and India. DNA taken from the shrouded remains of a man discovered in a tomb next to the Old City of Jerusalem shows him to be the earliest human proven to have suffered from leprosy.[8] In 1995, the World Health Organization (WHO) estimated that between 2 and 3 million people were permanently disabled because of leprosy at that time. In the past 20 years, 15 million people worldwide have been cured of leprosy.Although the forced quarantine or segregation of patients is unnecessary in places where adequate treatments are available, many leper colonies still remain around the world in countries such as India (where there are still more than 1000 leper colonies), China, Romania, Egypt, Nepal, Somalia, Liberia, Vietnam, and Japan. Leprosy was once believed to be highly contagious and ...

  • Discoid Lupus Erythematosus - Treatment Dr. Diane M. Hoss, MD discusses Discoid Lupus Erythematosus - Treatment. See more at PLEASE RATE AND COMMENT!!! Since DLE can cause irreversible scarring alopecia, the goal of therapy is to prevent the formation of new lesions and to treat early existing lesions before scarring occurs. DLE responds better to therapy than LPP. Patients with DLE should be counseled on sun protection and avoidance. Midpotent and superpotent topical steroids can be used to treat existing lesions. Intralesional triamcinolone, at doses of 5 to 10 mg are often effective at treating existing scalp lesions (lower concentrations should be used for facial lesions). I recommend against initial concomitant use of superpotent topical steroids and intralesional triamcinolone, as this combination can result in significant atrophy. If the patient is not developing new lesions, topical and intralesional steroids can be the mainstays of treatment. However, these treatment modalities do not prevent development of new plaques; this requires systemic therapy. Hydroxychloroquine, or Plaquenil, is the most effective systemic therapy with the least amount of toxicity for DLE patients. After obtaining a baseline complete blood count (CBC), liver function tests (LFTs), a G6-PD, and an eye exam by an ophthalmologist, start hydroxychloroquine at a dose of 400 mg a day. Repeat the CBC and LFTs about one month after initiating therapy, and then every 4 to 6 months. An eye exam should be performed by an ...

  • What Is Methemoglobinemia ? Methhemoglobin is an oxidized form of Hb which has reduced affinity for oxygen, thus oxygen is not released to tissues even it is not able to transport the oxygen. Oxygen- Hb dissociation curve is shifted to left. Hb is converted to MHb when ferrous ion is converted to ferric ion, so blood colour becomes blue or brown. Normal concentration is 0-3%. Types of Methemoglobinemia 1. congenital - due to deficiency of Diaphorase I, it can also be present in patients of pyruvate kinase deficiency. 2. Acquired- exposure to dyes, drugs like dapsone, nitrateds, antibodies like trimethoprim, sulphonamides. Nitrates in fertilizers may leak to ground may contaminate well water. Treatment is giving 1% methylene blue over 5 minutes and followed by IV flush of normal saline, but before giving methylene blue you must check G6PD deficiency because methylene blue is contraindicated in G6PD deficiency. Signs and Symptoms like breathing problems like shortness of breath, cyanosis, headache, fatique, loss of conciousness, patient may need to be ventilated. Here this patient drink colour thinner mixed with HOLI colours.

  • Bullous Pemphigoid Treatment Dr. Wendy Levinbook, MD discusses Bullous Pemphigoid Treatment. See more at PLEASE RATE AND COMMENT!!! For mild, localized disease, topical steroids may be effective. Oral antibiotics such as tetracycline, minocycline, or erythromycin with or without niacinamide may also provide good treatment results in select patients with localized or generalized BP. Sulfones, such as dapsone or sulfapyridine may also be tried. These have been reported to be effective in up to 44% of BP patients. However, the mainstay of therapy in patients who fail to respond to antibiotics or to dapsone is treatment with systemic corticosteroids. Adjuvant immunosuppressive therapy with azathioprine or cyclophosphamide may be necessary in patients with severe disease and may be steroid-sparing. Other therapies with reported benefit include plasmapheresis and itravenous gammaglobulin.
  • Dermatitis Herpetiformis Treatment Dr. Wendy Levinbook, MD discusses Dermatitis Herpetiformis Treatment. See more at PLEASE RATE AND COMMENT!!! Treatment with sulfones such as dapsone or sulfapyridine provide dramatic relief from the symptoms of dermatitis herpetiformis within hours to a few days of treatment initiation. In fact, dramatic improvement following the administration of these medications can be used to support the diagnosis of this disorder. Both drugs control, but do not cure the disease. Dapsone is the first choice of the two as it tends to be more effective. In addition, strict adherence to a gluten-free diet for at least 6 months will improve the intestinal and the skin lesions and will allow most patients to begin a decrease in sulfone therapy. However, the diet has to be followed for many months (often 2 years) before medications can be discontinued. Symptoms and lesions may recur in 1 to 3 weeks if a normal diet is resumed. Current evidence indicates that a gluten-free diet needs to be continued indefinitely and that it may be protective against gastrointestinal lymphomas. However, this diet is often difficult to sustain for long periods of time. An elemental diet (a diet without full proteins composed of free amino acids, carbohydrates, and small amounts of fats) may also provide significant improvement in skin disease activity, independent of gluten administration. However, similar to the gluten-free diet, an elemental diet is often difficult to maintain.

  • Dissecting Cellulites Treatment Dr. Diane M. Hoss, MD discusses Dissecting Cellulites Treatment. See more at PLEASE RATE AND COMMENT!!! Dissecting cellulitis is difficult to treat. Initial treatment is often with antibiotics directed against Staphylococcus species which are sometimes cultured from the drainage. Combination therapy with rifampin and cephalexin has been beneficial in anecdotal cases. Systemic corticosteroids can be used to calm down a marked inflammatory flare of the condition. Patients are often referred to surgery for drainage of inflammatory nodules and removal of fibrotic tissue. Isotretinoin and dapsone have been reported to be beneficial for some patients with the disease. I have one patient who responded well to dapsone. X-ray epilation and laser hair removal have also been reported to be beneficial in some patients.
  • Cicatricial Pemphigoid Treatment Dr. Wendy Levinbook, MD discusses Cicatricial Pemphigoid Treatment. See more at PLEASE RATE AND COMMENT!!! Treatment of cicatricial pemphigoid is based on the severity of disease and on the site of involvement. Mild or localized disease can be treated with topical steroid preparations. Dapsone may also be tried. However, in severe cases, systemic glucocorticoids are often necessary. For resistant disease, an immuosuppressive agent such as cyclophosphamide or azathioprine may need to be added. Cyclophosphamide provides better treatment results than azathioprine but it is more toxic. Because multi-organ involvement is typically seen with this disorder and because of the potential for severe complications, patients often need to be managed by a team of specialist physicians.

IMPORTANT SAFETY INFORMATION Do not use ACZONE® Gel if you are allergic to any of the ingredients in ACZONE® Gel or if you are younger than 12 years of age. Tell your doctor about all of your medical conditions, including if you have glucose-6-phosphate dehydrogenase deficiency. Like all medicines, ACZONE® Gel can cause some side effects. The most common side effects of ACZONE® Gel are dryness, redness, oiliness, and peeling of the skin being treated. When the active ingredient of ACZONE® Gel (called dapsone) is taken orally as a pill, it has been related to the abnormal breakdown of red blood cells (hemolytic anemia). If you have glucose-6-phosphate dehydrogenase deficiency, you may have a greater risk for lowering your hemoglobin level. However, using ACZONE® Gel on the skin is not expected to put enough dapsone in the blood to cause clinical symptoms of hemolytic anemia. You are advised to be alert for signs and symptoms suggestive of this type of anemia (sudden onset of: back pain, breathlessness, tiredness/weakness with daily activities, dark-brown urine, high fever, and yellow or pale skin). If you experience these signs and symptoms, stop use and call your doctor immediately. Use of benzoyl peroxide together with ACZONE® Gel at the same time may cause your ...
