Cytology-based screening programs, in use in developed nations, are not feasible in the developing world due to high cost, need for infrastructure, and trained cytologists. Mobile telemedicine allows medical consultations to be submitted via mobile phone, at low cost, without the need for a computer or Internet access. Telemedicine is best suited for visually-based diagnostics, and thus is ideal for use with cervical cancer screening using visual inspection with application of 4% acetic acid (VIA), a simple, inexpensive alternative to cytology-based screening. It is our goal to use mobile telemedicine to provide accurate diagnosis of precursor lesions, and appropriate triaging and implementation of therapy for women at risk for cervical cancer. Using the 5 Megapixel camera-enabled Samsung Soul U900 cell phone, clinicians in VIA see and treat cervical cancer screening clinics can capture high quality images of the cervix. Photos are then transmitted via the mobile phone to a gynecology specialist located remotely. We present the design and initial results of a prospective case control study to evaluate the accuracy and safety of remote diagnosis using photographic images of the cervix in HIV positive women in Gaborone, Botswana.
