  • Kidney Impairment with Byetta (March 2010) To help clinicians weigh the benefits and risks of the diabetes drug Byetta (exenatide), FDA is requiring that information about the risk of renal impairment be added to the drug's labeling. From April 2005 to October 2008, FDA received 78 reports of altered kidney function in patients treated with Byetta. Some of the patients had pre-existing kidney disease and other risk factors for developing kidney problems. Acute renal failure occurred in 62 patients and renal insufficiency in 16 patients. More than 6.6 million Byetta prescriptions were dispensed during this same time period, so these cases represent a small percentage of the total number of patients treated with the drug. Here's what the drug's labeling says. Byetta should not be used in patients with severe renal impairment or end-stage renal disease. For patients with moderate renal impairment, use caution when starting Byetta or increasing the dose from 5 mcg to 10 mcg. Monitor patients for signs and symptoms of altered kidney function, including increased serum creatinine, changes in urination, unexplained swelling, increases in blood pressure, lethargy, or dull ache in the mid to lower back. Consider discontinuing the drug if a patient's kidney function worsens while using it. Patients receiving Byetta should be told to watch for the signs and symptoms of altered kidney function and report these to their healthcare professional. Patients should also report nausea and vomiting, because these could lead to ...

  • "Know Your Heart Series", Diagnostic Testing, Blood Tests There are Fundamental Blood Tests that are important in order for the Cardiologist to assess an individuals heart health. These tests include a Lipid Panel, which measures Cholesterol, High Density Lipoprotein, Low Density Lipoprotien, and Triglycerides. The Complete Blood Count is of importance, in that it can indicate whether an infectious process is occuring in the body, via an abnormally high White Blood Cell Count. In addition, an abnormally low Hemoglobin/Hematocrit, can be consistant with Anemia. Anemia can induce Ischemia in the Heart, due to the fact that there is inadequate Oxygen transporting capacity of the Red Blood Cells. A Metabolic Panel is an important blood test, in that it includes the Heart Electrolytes, Potassium, Sodium, Calcium and Magnesium (sometimes Magnesium must be ordered seperately). In addition, the Metabolic Panel includes the Renal Function (Blood Urea Nitrogen/BUN, and Creatinine, Effective Glomerular Filtration Rate/eGFR). Abnormal kidney function effects the filtration of the blood, including electrolyte levels, which, if abnormal, could cause a Heart Rhythm Disturbances. Lastly, the Serial Cardiac Enzymes are important, in that the Troponin I is the most sensitive measure of damage to the heart muscle cells. The Troponin I is released upon the death of heart muscle cells (the Myocyte) into the blood-stream. The abnormally high Troponin I, and abnormal changes in the EKG, are clinically suggestive for a Myocardial Infarction. Also ...

  • New Warnings on Exjade (Aug. 2007) Novartis has notified healthcare professionals that new warnings have been added to the labeling for Exjade (defarasirox). Exjade is used to treat transfusional hemosiderosis, the chronic iron overload that can result from blood transfusions. Cases of acute renal failure, some of them fatal, have been reported in patients taking this drug. Most occurred in patients with multiple co-morbidities who were in advanced stages of their hematological disorders. Before beginning therapy with Exjade, all patients should have their serum creatinine assessed in duplicate to establish a baseline level, and then creatinine should be monitored monthly during treatment. Patients who may be at increased risk of complications should be monitored weekly during the first month of Exjade therapy or when the treatment is modified, and then monthly thereafter. These high-risk patients include the elderly, people with pre-existing renal disease or other co-morbid conditions, and those receiving drugs that can depress renal function. There have also been reports of cytopenias in patients on Exjade, some of them fatal. The relationship between the drug and these effects is uncertain, since most of the patients had pre-existing hematological disorders that themselves are associated with bone marrow failure. Blood counts should be monitored regularly and treatment interrupted in patients who develop unexplained cytopenia. Additional Information: FDA MedWatch Safety Alert. Exjade (deferasirox ...

  • New Zometa Dosing for Patients with Renal Impairments June05 Novartis has notified healthcare professionals about new warnings and dosing recommendations for the company's bisphosphonate drug, Zometa (zoledronic acid). Zometa is used to treat multiple myeloma and metastatic bone lesions from solid tumors, and hypercalcemia of malignancy. The changes are intended to improve the renal safety of Zometa. The revised labeling includes an additional warning regarding patients with preexisting renal impairment who are undergoing multiple treatments with Zometa and other bisphosphonates, because they may be at increased risk of renal deterioration. The new dosing recommendations are intended for patients with advanced cancer who also have reduced renal function, that is, those patients whose baseline creatinine clearance is 60 ml/min or lower. The revised labeling stratifies the recommended Zometa doses for patients with mild and moderate renal impairment according the baseline creatinine clearance rates. The company recommends that during Zometa treatment, serum creatinine be measured before each dose, and that treatment should be withheld when there's evidence of renal deterioration.

Somlyai 1) Semmelweis University Inst. Pathophysiology, Budapest, Hungary 2) HYD LLC. for Cancer Research and Drug Development, Budapest, Hungary Deuterium, a stable isotope of hydrogen, binds to oxygen to form D2O. D2O exist in the environment at 1/6700 of H2O (150 ppm) and is expected to have some biological effects. Several lines of evidences suggest that D2O inhibits insulin release from pancreatic islets. Very little or no data is available on the action of lowering D2O content of the cellular environment. Some experimental and clinical observations suggest that depletion of D2O has anti-mitotic effect in various tumor cells. Some clinical observations also suggest that depletion D2O interfere with glucose metabolism in diabetic patients. In our experiments we wanted to test the effect of removal of D2O on the glucose metabolism in sreptozotocin (STZ)-induced diabetic rat model. Diabetes was induced by a single ip. injection of 60mg/kg body weight of STZ. After 2 weeks, animals were randomly distributed into several groups to test the effect of D2O (25-150 ppm) on glucose metabolism in diabetic animals with or without 2x1 U/day insulin treatment. The following parameters were tested: serum glucose, -fructose amine, -HbAIC, -creatinine, -TBARS and -insulin; urine glucose, -creatinine ...
  • Creatine Versus Micronized Creatine For more info on creatine visit my site at : Detailed info: What's Really in Your Supplements? -- An Update on Creatine As some well informed readers might recall, I wrote an expose on the various impurities and contaminants found in certain creatines on the market a few years back. The article was called "What's really in your creatine?". The article took a close look at the large variations in the quality of different creatines and killed the long standing myth that "creatine is creatine." I expected the article to have an effect on the creatine market both from a wholesale and a retail perspective, and it did. As I predicted in that article, It also got me into hot water with many companies and individuals who sell less than pure high quality creatine. My exact words were "What I am about to tell you is not going to make me a very popular person with many supplement manufacturers. In fact, some of them are going to be down right pissed off at me." I recall the drunk owner of one well known supplement company coming up to me at an after hours party at a convention and slurring "Hey Brink, you cost me a lot of money with that article of yours" to which I responded "serves you right for selling low grade creatine!" I knew where he got his creatine and it was junk. The article examined several key impurities often found in low grade creatines such as Dicyandiamide, Creatinine, Dihydrotriazine, and Sodium. • Psoriasis Treatment Systemic Therapy Dr. James L. Campbell Jr., MD discusses Psoriasis Treatment Systemic Therapy. See more at PLEASE RATE AND COMMENT!!! Patients with psoriasis involving more than 20% of the body surface or who are very uncomfortable should consider systemic therapy. Systemic therapy is complicated and best managed by a dermatologist. A rotational approach to therapy minimizes long-term toxic effects from any one therapy and allows effective long-term management. Methotrexate is effective in unstable erythrodermic, generalized pustular psoriasis and extensive chronic plaque disease. It is effective for psoriatic arthritis and can be given orally, intramuscularly or subcutaneously in doses of 12.5 to 22.5 mg weekly. Close follow-up is needed; monitor complete blood cell count, liver function, and liver biopsy should be performed periodically. Beware of potentially life-threatening drug interactions with salicylates, many non-steroidal anti-inflammatory agents, trimethoprim sulfamethoxazole, penicillins, and others. Side effects include nausea, anorexia, fatigue, oral ulcerations, leukopenia, and thrombocytopenia, hepatic fibrosis or cirrhosis. Use caution in the elderly or patients with renal insufficiency. Cyclosporine is best used for severe inflammatory psoriasis and better yet for acute control only. The typical dose is 2.5 to 5.0 mg Once control is achieved, the dose is slowly tapered. Close monitoring of blood pressure is needed, as well as complete blood count ...
