  • Avoiding Medication Errors with Multiple Brand Names (April 2009) The Institute for Safe Medication Practices (ISMP) recently highlighted medication errors that can occur when a drug is marketed under more than one brand name, especially when one of those names is well established. ISMP describes a case where a female patient went to the emergency department with ischemic chest pain and ECG changes. One of her medications was Revatio, which had been prescribed to treat her pulmonary arterial hypertension. The ED physician reviewed the patient's medication list but did not recognize that Revatio contains sildenafil, the active ingredient in Viagra. The patient was given aspirin and sublingual nitroglycerin, and later was started on an IV nitroglycerin infusion for continued chest pain and elevated troponin-T. Using organic nitrates in any form, at any time, with sildenafil and similar erectile dysfunction drugs is contraindicated. Fortunately, this patient did not experience adverse effects or blood pressure changes because an internist quickly recognized the problem and stopped the infusion. ISMP cites other cases where patients received duplicate therapy after taking the same medication prescribed or dispensed under two trade names. These errors have occurred with bupropion (sold as both ZYBAN and WELLBUTRIN), finasteride (sold as PROPECIA and PROSCAR), and fluoxetine (sold as SARAFEM and PROZAC).   • Warnings for Bextra Strengthened (Feb 2003) In an earlier program, we talked about serious skin and hypersensitivity reactions associated with the drug Bextra (valdecoxib). Bextra is a Cox-2 selective inhibitor used to treat osteoarthritis, rheumatoid arthritis and dysmenorrhea. Now we want to tell you about strengthened warnings about these skin reactions, and also about cardiovascular risks associated with Bextra. Despite earlier warnings, FDA is still receiving reports of rare but serious skin reactions with Bextra, including Steven-Johnson Syndrome and toxic epidermal necrolysis, both of which can be fatal. So now the manufacturer, Pfizer, has added a boxed warning about this risk. Although other Cox-2 inhibitors and traditional NSAIDS also increase the risk of these skin reactions, the rate of these side effects appears to be greater for Bextra than for other COX-2 agents. So the labeling advises practitioners to watch for these skin reactions. Although they're most likely to occur during the first 2 weeks of treatment, they can occur any time during therapy. The labeling says to discontinue Bextra at the first appearance of a skin rash, mucosal lesions, or any other sign of allergic reactions. Bextra is contraindicated in patients who are allergic to sulfonamides. Rifampin has been contraindicated with saquinavir, because the rifampin will reduce plasma levels of saquinavir. A study was recently performed to see whether boosting saquinavir with ritonavir would overcome this problem. But when healthy volunteers in the study were given rifampin along with the combination of saquinavir and ritonavir, 39 percent developed significant hepatocellular toxicity. As a result, Roche Pharmaceuticals is cautioning that rifampin should not be given to patients who are also receiving a combination of saquinavir and ritonavir.

This product is a long-acting cough suppressant containing hydrocodone, a narcotic antitussive, and chlorpheniramine, an antihistamine. Overdoses of hydrocodone can result in respiratory depression and death. FDA has reviewed over a hundred adverse event reports associated with this product. Twenty four reports were in children under the age of six, nine of whom died. Tussionex is not approved for children under six because of the risk of respiratory depression and death. Even though it is specifically contraindicated in this age group some physicians are prescribing this product for them. Also, family members may take a prescription for an older family member and give it to younger children. Irrespective of the child's age, parents are also running into trouble when they try to measure the dose. For example, they may be using ordinary teaspoons, which can vary in size. Pharmacists can contribute to the problem if they mis-state the dose on the prescription label. In one case, the physician's order of 2 mL was written as two teaspoons on the label. Another problem with any long-acting or extended-release medication is that patients or caregivers may be tempted to use it more often than it is prescribed. It is available in several formulations, including syrups, suppositories, tablets or injectables. Practitioners should also be cautious when using promethazine in older children because respiratory depression can also occur in this age group, even when the dose has been calculated according to the child's weight. Ultrasound Micro-bubble Update on Cardiopulmonary Reactions with Ultrasound Micro-bubble Contrast Agents: FDA is alerting healthcare professionals about labeling changes for ultrasound micro-bubble contrast agents, which are sold as Definity (Perflutren Lipid Microsphere) Injectable Suspension and Optison (Perflutren Protein-Type A Microspheres for Injection). These products are used during echocardiography to enhance a cardiac image. The revised labeling continues to highlight the risk of serious cardiopulmonary reactions, either while the contrast agent is being administered or within 30 minutes afterwards, especially in patients with pulmonary hypertension or unstable cardiopulmonary conditions. However, based on a review of published reports and information from physicians, FDA has now determined that the benefits of the diagnostic information provided by Definity or Optison may outweigh the risk of cardiopulmonary reactions, even among some high-risk patients. As a result, these products are no longer contraindicated for patients with unstable cardiopulmonary status, including those with unstable angina, acute myocardial infarction, respiratory failure, or congestive heart failure that has recently worsened. The boxed warning points out that in these high risk patients, it is important to monitor vital signs, electrocardiography and cutaneous oxygen saturation while administering the contrast agents and for at least 30 minutes afterwards.

Meridia is used to manage obesity. In November 2009, an early communication from FDA reported preliminary study results suggesting that patients on Meridia had a greater frequency of cardiovascular events than those not taking the drug. Additional data from the study has shown that the excess risk occurred in patients with a history of cardiovascular disease. They should also discontinue Meridia in patients who do not lose at least 5 percent of their baseline body weight within the first three to six months of treatment, because further treatment is not likely to be effective and exposes the patient to unnecessary risk. Patients using Meridia should talk with their healthcare professional about whether to continue using the drug.

This is a classic case of a behaviorally fragile autistic patient with medical issues complicating treating the behavioral issues. (ie...use of anti-psychotics for extreme self injurious behaviors contraindicated due to seizure disorder) 5-htp has shown some improvement in mood, but behaviorally fragile autistics (as noted in other research) are notorious for having many good days, and then, with the slightest trigger, such as a loud noise or mild cold, the behavior erupts into a nightmare of constant medical and behavioral interventions, until the patient returns to some kind of homeostasis. This can require multiple, round the clock interventions. It is absolutely critical to eliminate any and all underlying medical issues that can alter behavior or seizures thresholds. For instance, if you eliminate the medical issues triggering SIB, than you can deduce that the SIB is being triggered from something else, often a combination of things.   • New Drug Approved for Parkinson's Disease FDA recently approved a new drug to treat Parkinson's disease, called Azilect (rasagiline). Azilect is an MAO type-B inhibitor that blocks the breakdown of dopamine in the brain. Azilect was approved as a single therapy for treating early Parkinson's disease, and also in combination with levodopa in treating advanced cases. As with many other MAO inhibitors, patients taking Azilect must avoid foods rich in tyramine such as aged cheese and red wine, and also medications containing amines, including many cough and cold remedies. Combining Azilect with these substances can trigger a hypertensive crisis which could be fatal. Like most other medications for Parkinson's, Azilect can cause dyskinesias, hallucinations and hypotension. Azalect is contraindicated in combination with several other drugs, including dextromethorphan, meperidine and other ***gesics. Melanoma has been diagnosed in a small number of patients treated with Azilect. Parkinson's disease itself appears to increase the risk of melanoma, so it's not known at present whether the drug further increases the risk. The manufacturer will perform a postmarket study to address this question. In the meantime, the product labeling will recommend that patients undergo periodic dermatologic examinations.

Unlike disorders such as bullous pemphigoid or dermatitis herpetiformis where there is a large inflammatory component in addition to the immune component, anti-inflammatory drugs have no known benefit in the treatment of pemphigus vulgaris. It is generally agreed that patients with only limited initial disease need as much intensive therapy to induce a remission as patients with extensive involvement. Prednisone is the standard initial therapy unless contraindicated. Most patients obtain a complete or almost complete clinical remission within 8 to 10 weeks and then the dose can be tapered gradually. Low doses are then maintained for several years. Other immunosuppressive drugs such as azathioprine or cyclophosphamide may be added if prednisone fails to induce a remission or if the patient develops serious corticosteroid complications. Mycophenolate mofetil, methotrexate, and cyclosporin are added to prednisone as adjuvant therapies less frequently. Other treatments that have been utilized include intravenous pulse methylprednisolone, intravenous pulse cyclophosphamide, adjuvant intravenous gammaglobulin, immunoablative intravenous cyclophasphamide without stem cell rescue, intramuscular gold, and extracorporeal photochemotherapy.
