Atrial Septal Defect with Interstitial Lung Disease Earlier you have seen several of awake cardiac surgeries in our series. This is another interesting patient a 45 year old male with interstitial lung disease along with atrial septal defect. He also had hypertension and diabetes. He had progressively deteriorating symptoms and high Pulmonary Artery Pressures but a persistent left to right shunt. His gas exchange was fairly well preserved. Operating him was a challenge as if we subjected him to general anesthesia and mechanical ventilation, it would have been difficult- may be impossible- to wean him off ventilator- because of his pre-existing lung disease. Hence it was decided to operate him under thoracic epidural anesthesia with patient being awake during the surgery. We took up the challenge and operated him. We succeeded in doing the surgery awake and the patient recovered well with improved oxygenation and other parameters. We put in a fenestrated valved patch which would allow flow of blood from right atrium to left atrium at a later stage if his right sided pressures increased due to his lung disease. We did the surgery at Indore, MP India. Chulalongkorn Hospital's staff tell fear from reds. Doctors and medical staff have spoken of their distress and fear after red shirt protesters raided Chulalongkorn Hospital last week. he red shirts' raid on Thursday forced the evacuation of more than 100 patients from the hospital and triggered public condemnation. Medical staff told a forum yesterday organised by the Thai Journalists Association that they were now living in fear. The protesters agreed yesterday to allow access to the hospital's emergency unit in the face of growing public criticism. Thammasak Thawitsri, an anaesthesiologist at the hospital, said staff were worried about their safety. ``We didn't know what to do, the hospital looks like a desolate place. I cannot hold back my tears,'' Dr Thammasak said. Supa*** Sritangsirikul, a C7 nurse, said staff felt unsafe. Before they could enter the hospital, they had to pass through a checkpoint manned by red shirt protesters who demanded bag searches. At one point the nurses heard continued gunshots and crouched in fear, she said. Then, a patient beeped for help and the nurses had to crawl into the patient's room to attend to him. ``Safety is the basic right of people,'' Mrs Supa*** said. ``If medical professionals feel unsafe, they will lose concentration and cannot deliver their services. ``We maintain our neutrality. Colton Chunn - Please pray for Cole, he's battling lymphoblastic lymphoma Cole's ANC on Thursday was 3800 so they have raised his chemo again. He received the dreaded vincristine and began steroids. The lumbar puncture went well. We had the anaesthesiologist I don't like. I told her that he doesn't take much to go out. This time it went well!!!!! When we got home Cole asked me if the doctors did surgery on his back because it hurts. I told him they did not do surgery but he took off his shirt and asked me to look for a mark. I told him there was a little dot because they gave him a pokie. He understood that well enough. Saturday Cole was screaming at the top of his lungs at everyone then throwing a fit. I thought he was going to make me crazy. Then it dawned on me that Cole is on steroids and he is the one that's crazy. LOL The whole steroid rage really stinks. With Cole being a week behind on his schedule from being sick he will receive vincristine and start steroids again in 3 weeks instedad of 4. I hope this is not too bad on his legs but we will see. The babies a boy and a girl and their mothers are reported to be doing well. They were born within the magnitude 6 aftershock that struck early in the morning. A patient suddenly arrived on whom we had to do a caesarean, and another who we thought we would have to do a caesarean, explained Arthur Halvorsen, a Norwegian Red Cross anaesthesiologist. We took the first mother in to the operating theatre and managed to deliver the baby in a caesarean. This kid is now doing very well. The other mother who was brought in actually ended up giving birth naturally while we were doing the first caesarean. A large number of those coming here are having to have amputations since their wounds are so infected, explained Norwegian Red Cross surgeon Brynjulf Ystgaard.
